Client Intake Form (Standard For Massage Therapists)

This is where my client intake form is located.

If you have come to this page, you have probably scheduled a first appointment.

Or you might also be a massage therapist wanting to use my client intake form.

You are allowed to use my client intake form in any way you like.  All I ask is that in exchange, you check out http://www.MusicOfMassage.com (opens in new window) and see if it will be a good service for you to use with your clients.

Please print out this intake form and fill it out before coming to receive bodywork for the first time.  

That way, time will be saved for doing what you really want, which is to receive a great massage!

Why do you ask the questions on the client intake?
The Client intake is my way of getting to know who you are as an individual.� It asks questions about your medical history and about your activities, so that I can get an understanding of what may be best for the individual bodywork I will do with you.� Please fill it out as completely as possible so that I can give you the best massage possible.

 

Do I need to bring a client intake every time I receive bodywork from you?
No you don't.� On the first appointment, I will do my best to gain an understanding of your history and current situation, and then will keep your client intake on hand for future reference.� I do ask, however, that when you come to receive bodywork from me and have had changes from what you initially input on your intake form, that you will let me know.� (i.e.� If you had surgery since the last time you received bodywork, or started a new medication.)
